EDWP Elderly and Disabled Medicaid Waiver Program (EDWP)

CCSP (the Community Cares Services Program) and SOURCE (Service Options Using Resources in a Community Environment) are both part of the Elderly and Disabled Waiver Program (EDWP). Both assist individuals who are elderly and/or functionally impaired to remain in their homes and/or communities.  For eligible consumers, EDWP offers community-based services as an alternative to nursing facility placement. CCSP is a traditional service model, and SOURCE is an enhanced model which links to primary medical care.
