Options Counseling
Returning home after a stay in a nursing home or other long-term care facility is a big decision. A Certified Options Counselor can meet with you and your loved ones to discuss your preferences, needs, and options to help you successfully move back home.
Money Follows the Person (MFP)
Money Follows the Person is a Medicaid program that provides eligible individuals with the support they need to move back into their homes and communities. Services include home modifications, household supplies, medical equipment, and life skills coaching.
MFP Eligibility
- Must be in a nursing facility for at least 60 consecutive days
- Must apply for and receive at least one day of nursing home Medicaid
- Must have community Medicaid
- Must enroll in Medicaid Waiver
Nursing Home Transitions
Much like the Money Follows the Person program, the Nursing Home Transition program supports clients who are at risk of long-term placement in nursing homes. This program is a non-Medicaid program and is specifically for individuals who are 55 or older. Services may include one-time help with rent, utility deposits, medicals supplies, and environmental modifications.
Nursing Home Transitions Eligibility
- Must be in a nursing home or short-term rehab for at least 20 days
- Must be 55 years of age or older
- Must be a US citizen or have legal status
- Income must be below $6,175 per month
